How to use
- Fill whatever you know. Employee name, CNIC, personal number and office are remembered across every form on this site.

- Get it signed and stamped by the DDO / competent authority before submitting it.
- Info Type and Field ID codes come from the AG Sindh payroll manual — enter exactly what your DDO office specifies.
- Use PAY03 instead when the same amendment applies to several employees.
